Skilled Care Services:
Skilled Care services are performed by a Registered Nurse (RN) and/or a Licensed Practical Nurse (LPN) who specialize in home health. They will work closely with your doctor to ensure that your recovery goals and needs are met.
This includes:
- Skilled observation and assessment
- Disease management (COPD, Diabetes, CHF, HTN, Stroke, Cancer)
- Wound/ostomy management
- Anticoagulant monitoring
- Indwelling catheter management
- Home Infusions
- Surgical site care/wound care
- Pain management
- Medication management
- Ongoing support and review of post-surgical instructions
- Short-term post-operative care
